Facial Surgery: What is It and How Does It Work?

Facial surgery, also called orthognathic or jaw surgery, corrects the position of the upper jaw, the lower jaw or both, in cases where the underlying skeleton sits in a way that braces or aligners alone cannot fully address. It is used for larger jaw discrepancies, facial asymmetry, and bites that are driven by the position of the bones rather than the teeth. At ONE ORTHODONTICS, facial surgery is planned and delivered as part of a combined orthodontic and surgical pathway, so the orthodontic and surgical stages fit together from the start.

A combined orthodontic and surgical pathway

Most jaw surgery is carried out in stages. Orthodontics first prepares and positions the teeth so the jaws can meet correctly once they are moved. Surgery then repositions the jaw or jaws. A further phase of orthodontics settles the final bite. Because your orthodontist and surgeon plan the case together at our clinic, each stage is set up with the final result in view, and you are looked after by the same team throughout.

Your care is led by Dr Chris Donaldson, Consultant Orthodontist and Clinical Director, who is dual-qualified in medicine and dentistry. Surgical assessment and treatment are provided in-house by Mr Ash Messiha, Consultant Maxillofacial Surgeon, who is also dual-qualified in medicine and dentistry and whose practice includes corrective jaw surgery, facial asymmetry and jaw-joint problems. Planning the orthodontic and surgical sides together, under one roof, means both specialists work to the same plan for your face and your bite.

Before any surgery, the jaws are planned in three dimensions. We carry out this 3D virtual surgical planning in-house, using our own planning software, so your case is prepared by the specialists looking after you. Working from 3D imaging of your face and jaws, the movements are rehearsed virtually, and you can see a simulation of the likely result before you commit. This is an estimate to guide the plan and set expectations, not a guarantee of the final outcome. Where it improves accuracy, the agreed plan can be transferred to surgery using 3D-printed guides, which are produced externally to our specification.

The appropriateness of facial surgery for treatment can only be determined by a clinical assessment. In general, it is considered for:

Significant upper or lower jaw discrepancies

Facial asymmetry

Open bites or deep bites driven by the position of the jaws

Difficulty with biting, chewing or lip closure related to jaw position

Some cases involving the airway, assessed alongside our airway and sleep pathway
